Paper Forms

These forms are PDF files that can be printed and filled out. Paper copies are also all avaliable in Morrill 205.

Excessive Absence Report for the Advisor
This form is used to inform a student's advisor of excessive absences, so the advisor can check whether a student is still enrolled and counsel them about attendance and drop dates if possible. Send this form via capus mail to the address listed on the form. You will usually receive a report back stating whether the student has dropped the course or not.

Excessive Absence Report for the Registrar
This form is for reporting all the students in a given class who are not attending. The Registrar's Office uses this form for verifying when a student last attended when processing withdrawl forms and for financial reporting.

Request for Change of Final Grade
This is the form used to change a student's final grade, including grades of "I."
